Surrogacy Cost Breakdown FAQs

Most online surrogacy calculators only estimate base compensation or a limited portion of the journey. A comprehensive calculator should include medical, legal, agency, and support costs to give a realistic total.

The SurrogateFirst cost calculator is designed to reflect the full scope of a gestational surrogacy journey, including IVF procedures, surrogate compensation, legal agreements, insurance, and additional allowances. This provides a more accurate planning tool compared to simplified estimates that exclude key expenses.

Use our Surrogacy Cost Calculator

A complete surrogacy cost breakdown usually includes several categories:

  • Surrogate compensation (base pay and additional allowances)
  • Medical expenses (IVF procedures, medications, screenings, prenatal care, delivery)
  • Legal fees (contracts, independent legal representation, parentage orders)
  • Agency fees (matching, coordination, case management)
  • Insurance and escrow management
  • Reimbursements and support costs (travel, lost wages, childcare, wellness support)

In the United States, the total cost of a gestational surrogacy journey in 2026 typically ranges between $100,000 and $200,000, depending on factors such as surrogate compensation, medical complexity, legal requirements, and agency support.

Some international options may appear more affordable, but they often involve different legal frameworks, travel requirements, and additional risks that should be carefully considered.

Read more about the 2026 surrogacy cost overview here.

Medical costs in a surrogacy journey are primarily driven by IVF treatment and the surrogate’s pregnancy care. In the United States, a single IVF cycle typically ranges from $15,000 to $25,000 including medications, depending on the clinic and treatment plan.

  • IVF procedures (egg retrieval, fertilization, embryo transfer)
  • Medications (hormone stimulation and support)
  • Monitoring and testing (ultrasounds, blood work)
  • Laboratory services (e.g. embryo culture, genetic testing if applicable)
  • Surrogate medical care (screening, prenatal care, delivery)

Because success is not always achieved on the first attempt, multiple IVF cycles may be required, which can significantly increase total medical costs.

It’s important to check with your insurance which of those costs may be covered.

Legal fees for a surrogacy agreement typically range from $8,000 to $15,000 in the United States, depending on the complexity of the arrangement and the state in which the contract is executed.

You can learn more about the surrogacy legal agreement in this article.

Legal fees in surrogacy are designed to ensure that both the intended parents and the surrogate are fully protected and independently represented.

  • Contract drafting and negotiation
  • Separate attorneys for each party
  • Establishment of legal parentage (such as pre-birth or post-birth orders)

Learn more about surrogacy contracts

SurrogateFirst strives to be as transparent as possible and therefore outlines upfront included the most major cost categories.

We noticed that on independent/other providers’ journeys, those costs get the most oversight:

  • Cycle-related changes (e.g. canceled or repeated embryo transfers)
  • Medical contingencies (such as complications or extended care)
  • Additional allowances (bed rest, multiples pregnancy, or travel adjustments)
  • Insurance gaps or changes in coverage
  • Legal updates or contract amendments

A well-structured surrogacy plan anticipates these scenarios and includes clear terms and financial protections to reduce uncertainty.

Packages can also help alleviate some of the risk.
